These products must consistently maintain quality in accordance with ISO 9001:2015 standards. PT SKI aims to reduce defective products to 0.2% across all products. However, in 2023, there was a product with the highest defect rate, reaching up to 14.55%, specifically the LDAM 000400-0. This defect rate exceeds the target, resulting in losses for the company. Therefore, a quality improvement initiative was undertaken for LDAM 000400-0 product using Six Sigma methodology with the DMAIC (Define, Measure, Analyze, Improve, Control) approach to reduce the defect rate. The quality improvement project began with the formation of a quality improvement team in the Define phase. Subsequently, process stability and capability measurements for LDAM 000400-0 product were conducted, resulting in an average sigma value of 2.94 in the Measure phase. In the Analyze phase, interviews with relevant stakeholders were conducted using the Delphi method to identify causing defects factors and sub-factors. A significance analysis was performed to determine the factors significantly impacting quality, followed by regression modeling to predict responses under the identified factor conditions. These results formed the basis for proposed solutions in the Improve phase. Proposed solutions included etching time standards, inspection standards, and process etching rack hangers. Standardization of the new processes was carried out through the creation of a Standard Operating Procedure. This quality improvement project is estimated to increase the sigma level to 4.3. text
