Turbine blade failure is a critical problem that can result in large losses, both operational and financial. The aim of this research is to identify the factors that cause failure, analyze the damage mechanism, and provide recommendations to prevent similar failures from occurring in the future. The research methods used include visual inspection, metallographic analysis, hardness testing, and chemical composition analysis. Turbine operational data is also studied to understand the working conditions and loads faced by the blade. The results showed that the failure of the L-2 blade was caused by a combination of material fatigue, corrosion and mechanical vibration factors. Metallographic analysis revealed the presence of microcracks on the blade surface caused by material fatigue. In addition, a corrosive operating environment also accelerates the material degradation process. Recommendations include improving predictive maintenance programs, using materials with better corrosion and fatigue resistance, and improving blade design to reduce mechanical vibration. Implementation of these recommendations is expected to increase the reliability and service life of turbines, as well as minimize the risk of failure in the future. text
