The impact of implementation of decentralization in Yogyakarta causes a decrease of 44.5% for laboratory examination in District Health Laboratory Yogyakarta. To improve the performance of health laboratory services in order to meet customer satisfaction, District Health Laboratory Yogyakarta applied quality management system ISO/IEC 17025 Objective: To investigate the effect of variables in the implementation of Total Quality Management (TQM) in Yogyakarta, Health Laboratory consisting of: the commitment and support of top management, employee empowerment, education and training, customer focus, continuous improvement, cooperation with suppliers, information and analysis, and benchmarking performance against organizational performance. Methods: We applied descriptive cross sectional method using quantitative and qualitative data. The population of this study was all employees, while the study sample was 53 respondents with Proportional Random Sampling as a sampling technique. For qualitative data collection we conducted interviews with the leadership element BLK Yogyakarta as a respondent. Results: The simultaneous implementation of TQM had significant effect on organizational performance (p = 0.000 F = 14.953). Magnitude of the effect was 62.8% influenced by elements of TQM, while the remainder by other factors. Top management commitment and support have the value of p = 0.005 and therefore contributes significantly, employee empowerment p = 0.794 (no significant influence), education and training p = 0.024 (have significant influence), customer focus p = 0.420, which means not significant, p = 0.005 continuous improvement, collaboration with suppliers which means p = 0.584 significant influence, information and analysis of p = 0675 which means no significant influence, and performance comparison p = 0.037 (significant influence). Conclusion: There was a significant influence on the simultaneous implementation of TQM in Health Laboratory Yogyakarta on organizational performance. Partial implementation of TQM variables that significantly influence the performance of the organization is the commitment and support of top management, education and training, continuous improvement and performance benchmarking.
