The types of equipments utilized are <br /> <br /> <br /> articulated dump truck AD-30 and AD-55. Production target in year 2011 is 90k dmt/day that required availability of the haul truck with one of the way by reducing major components failure below the target life, major components consists of torque converter, engine, transmission and axle. Failures occur on major components causing maintenance cost of US $10,260,008 and contributing production opportunity loss of <br /> <br /> <br /> US $8,672,470. Using 5 Why’s root cause analysis, 7 factors that affected major component failure below the target life are obtained which are lack of PM and daily service quality, lack of major component repair quality, lack of product training from vendor, un-optimal <br /> <br /> <br /> PM checklist, lack of mechanic's training and on the job training (OJT), lack of final inspection from supervisor, lack of awareness from mechanic. Best alternative solutions for each root cause are: improve quality of PM final inspection and daily service, communicate with vendor (supplier) to improve major <br /> <br /> <br /> component repair quality, communicate with vendor and Quality Management Services (QMS) department to conduct special training regarding C-Series type engine, create PM checklist according to major component group, include mechanics to attend training conducted by vendor, improve mechanical technical knowledge on supervisors, develop performance management and mechanic scorecard. <br /> <br /> <br /> By implementing the solution alternatives it is gained production opportunity profit of US $11,523,602. text
