To prevent the prediction, Underground (UG) Mine Expansion Projects are accelerated. There are three main UG mining equipments which are UG Drill, UG Loader and UG Truck. UG Maintenance Department needs to provide 83% availability as target for UG Drill. Axera 8 Drill is utilized for main tunnel development at UG Expansion Projects. The tunnel size that can be developed by Axera 8 Drill cannot be done by similar drill equipment such as Axera 7 Drill. Within January 2010 to March 2011 availability of Axera 8 Drill is trending down and in some period the availability is under the target. This ongoing condition will contribute to USD 22.5 million production loss opportunity. There are some factors affecting equipment availability such as PM quality, daily service quality, breakdown repair quality, predictive maintenance quality, spare part availability and equipment damaged. Based on case study on Axera 8 Drill using 5 Why’s root cause analysis method, it is obtained that lack of enforcement and supervision to mechanics, lack of product training, inadequate engine specification, no daily service schedule, too many items in daily service schedule are top five root cause that will contribute to USD 18.36 million production loss opportunity. Proposed solutions are 1) upgrade Staff Performance Appraisal (SPA) for supervisors and & Non-Staff Performance Appraisal (NSPA) for leaders, 2) conduct Axera 8 product training for mechanics, 3) upgrade Axera 8 engine 4) create daily service schedule for drill equipment 5) create a simple and comprehensive daily service sheet for drill equipment. By implementing the proposed solutions and investing USD 1.14 million, it is expected that USD 11.87 million production improvement opportunity can be obtained. This benefit is exclude other benefit which is the lower equipment maintenance cost causing by the improvement of equipment availability. <br /> <br /> <br /> Dengan melaksanakan usulan solusi dan investasi sebesar 1,14 juta dolar Amerika, diharapkan peningkatan kesempatan produksi sebesar 11,87 juta dolar Amerika bisa didapatkan.
